Comment(s): The Renter agrees to:
•Deposit of $50.00 paid via credit card. Facilities used must be left in good and clean condition. If these conditions are not met to the satisfaction of the Parks Department staff, your deposit will be forfeited.
•Full payment must be received at the time the reservation is made.
•No animals are allowed in the City of Oldsmar parks, except in designated “pet areas”.
•No alcohol is permitted in the City of Oldsmar parks.
•The reservation fee is for the shelter only, no other park amenity (i.e., Cypress Forest Park is for the shelter, not the use of the water facilities).
•Tents, pop-ups, etc., of any size, are strictly prohibited at the Cypress Forest Park.
•Reservations can be rescheduled or canceled, with a full refund, no later than 14 days before the reservation date.

RELEASE OF LIABILITY AGREEMENT
I/We realize that the City of Oldsmar carries no medical insurance covering participation in these rental activities. I/We agree to release the City of Oldsmar from any liability of any kind for any and all damages arising out of any loss or injury resulting from participation in any activities of any kind in any way connected to the rental of the City of Oldsmar facilities/equipment. This release includes a release for any and all losses or injuries arising while utilizing this rental facility. The City of Oldsmar is not responsible for any injuries to the person(s) utilizing or attending a function and cannot be held liable. I/we agree to hold harmless the City of Oldsmar for any injuries or damages occurring as a result of the use of the rental facility or any activity sponsored by me at the rental facility. I understand that the Oldsmar Leisure Services Department has the right to assess additional charges if facility, equipment, or any privilege is abused, or if more equipment than specified is used. I also understand that my groups will be asked to vacate the premises immediately if any conduct is deemed unsatisfactory and all charges will be forfeited. I agree to abide by the building policies and restrictions as found in Policy Governing use of the City of Oldsmar facilities.*Failure to clean the pavilion after your rental will result in a forfeit of your deposit.
